Criminal Records
In Wisconsin, criminal history records are public records, provided the records are not sealed and the subjects of the searches are not juveniles. The Wisconsin Department of Justice, through the Criminal Information Bureau (CIB ), provides name-based and fingerprint-based criminal history checks. Employers, landlords, and residents of Dodge County may submit name-based requests by completing the Single Name Record Request form or Multiple Names Record Request form. Submit the applicable form, along with a check or money order made payable to the Wisconsin Department of Justice, and a self-addressed postpaid envelope to:
Criminal Information Bureau
Attn: Records Check Unit
P.O. Box 2688
Madison, WI 53701-2688
The Criminal Information Bureau charges $7 for each name-based search. If paper copies are also required, the office charges an additional $5 for each copy requested. Interested persons can also pay the total fees with their online billing account numbers. Note that an extra $3 DHFS fee applies to all caregiver requests.
The Department of Justice provides an online criminal history records request portal known as Wisconsin Online Record Check System (WORCS). Register by creating a user account and a billing account to be able to log in and complete a name-based request for criminal history records. The Wisconsin DOJ emails the search results to requesters. There is a $7 service fee for a general request type and a $10 fee for a caregiver request type. Pay an additional $5 if certification and notarization are also required. Note that certified copies cannot be emailed. As such, they must be sent by regular mail.
It is also possible to use the WORCS portal without registration. However, unregistered users must write down their order reference numbers (seen at the bottom of the receipts) to be able to retrieve results at later dates. Contact the office at (608) 266-5764 for additional queries.
For adoption and immigration purposes, eligible persons can apply for their complete fingerprint-supported criminal history checks and FBI criminal record checks. To request these records, visit any law enforcement agency in Dodge County to obtain and roll a fingerprint card. Send the rolled fingerprint card along with a Single Name Record Request Form to the Criminal Information Bureau’s mail address listed above. Each fingerprint-supported state check costs $15, while an FBI check costs $13.25. Include $5 for each additional paper copy.
